What can I expect on my first visit?

Your first visit will be customized to meet your specific needs and address any concerns or questions you may have for Dr. Cohen. During this time, you will have the opportunity to discuss the most appropriate treatment plan tailored specifically to your mouth with Dr. Cohen, ensuring that you conclude your visit with confidence and that you are completely comfortable with the procedure(s) needed to achieving that beautiful and healthy smile.

In order to best meet your needs and provide you with the most comprehensive treatment plan, please assist us by providing the following information to your first visit:

  • Your referral form and any x-rays your referring dentist or dental specialist has provided.
  • A completed copy of the registration form – this form is available to you under the patient information tab of this website.
  • A list of medications you are currently taking.
  • The last date of your physical evaluation from your physician.
  • Dental insurance forms or insurance cards (if you have dental coverage).

Dr. Cohen will complete a thorough evaluation of your medical and dental health history as well as perform a comprehensive intra- and extra-oral full mouth evaluation including an assessment of your teeth and the surrounding structures that support your teeth (gums, jaw bones, ligament, tooth root). Dr. Cohen will also review dental x-rays either given to us by your referring dentist/dental specialist or x-rays necessary to create the appropriate treatment plan to meet your dental needs.

Your initial visit will end with a question and answer session discussing various options along with their respective benefits and drawbacks, instructional audio and visual aids for further comprehension of your customized treatment plan, a sedation interview (upon request), and pre-operative instructions on how to prepare yourself prior to the following visit to ensure a positive and comfortable experience with us.

Please allow approximately 60-90 minutes for your initial visit.

All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment and every appointment thereafter.

To provide our patients with the utmost professional and enjoyable experience in our office, we ask that appointment cancellations be made 48 hours in advance in order to avoid any charges. This time is reserved exclusively for you and the doctor in order to best meet your dental needs.
